PHILOSOPHY OF HEALTH SERVICES
Integrating all aspects of well-being, the communities focus on the management of care across all domains in collaboration with the resident and other health providers to maintain or improve health and self care ability, to ensure appropriate interventions to manage the efforts of chronic disease or disability, to coordinate more aggressive medical treatment for episodes of acute illness, and to assist residents in maintaining a high level of functioning.
Primrose Retirement Community staff will provide and coordinate for residents’ assistance with health services, in a manner which upholds the dignity, choice, privacy, independence and individuality of each resident, and as indicated on the resident's Service Plan. Qualified staff will provide service in accordance with the community's policies and procedures, applicable regulations and established standards of practice.
The community promotes a multidisciplinary approach to health service planning and coordination with active involvement by the resident and his/her responsible party as indicated. Health service planning will strive to maintain or improve resident functioning, utilize resources appropriately, and maximize resident decision-making opportunities and education.
